  • FT - 6am-6pm or 6pm-6am

Position Summary:

Responsible for the direct careof those we serve and supervision of staff, in accordance with the NursePractice Act in the state in which employed as well as ministry policies andprocedures. Functions as a member of the interdisciplinary team.

KeyResponsibilities/Essential Functions:

• Performs delegated residentcare tasks in a timely and efficient manner

• Contributes to continuity ofcare and effective communication by documenting and reporting care provided andchanges in the resident’s condition to the appropriate person in a timelymanner.

• Anticipates, recognizes,reports and responds to changes in resident/patient condition.

• Implements interventionsbased upon the physician's medical plan and standards of care includingmedication and treatments with knowledge of therapeutic intent.

• Assists residents andfamilies with care planning, education and referrals.

• Follows regulatory andministry standards, policies, procedures and precautions in the performance ofall job duties to ensure a safe work environment.

• Responsible for residentsafety, identifying safety hazards and emergency situations and initiatescorrective action as appropriate.

• Assumes responsibility forcompliance with federal, state and local regulations within the assignedneighborhood.

• Provides support with Activities of Daily Living asneeded.

• May perform other duties asassigned or requested.

Management Responsibilities:

• Interprets existing policiesand procedures to certified nursing assistants, certified medication aides,residents, and families.

• Supervises staff, evaluatesthe provision of direct resident care, and initiates corrective action asnecessary.

• Follows staffing guidelinesand make nursing assignments according to qualifications of staff andcharacteristics of the resident population.

• Participates in the employeeevaluation process by providing information on the clinical performance ofcertified nurse assistants and certified medication aides to the DON or DCS.

• Monitors staff compliance with infectioncontrol, personal protection equipment, blood borne pathogens, hazardousmaterial handling and fire safety.

Job Requirements


Education and/or Experience

• Completion of an accreditedLPN program.

• A minimum of one yearexperience as a LPN preferred

• Supervisory experiencepreferred.

• Possesses working knowledgeof State and Federal regulations.

Technical Skills

• Demonstrates proceduralcompetencies for assigned tasks.

• Working knowledge of basiccomputer systems in order to complete data entry required.

• Ability to navigate theinternet and Via Christi intranet required

Certificates, Licenses,Registrations

• Valid license as a practicalor vocational nurse in the state in which employed required.

• CPR Certification requiredfor LTC upon hire, or obtain within thirty (30) days of employment.

• CPR Certification preferredfor AL.

Physical Requirements:

• Frequently required to pushand pull items, such as resident/patient beds, requiring a force of up to 75pounds.

• Frequently required to carryor lift items weighing up to 25 pounds at waist height. Required to obtainassistance of another qualified employee when attempting to lift objects over25 pounds.

• Frequently required to assistin maneuvering resident/patients weighing 100-400 pounds and occasionallyrequired to assist in maneuvering resident/patients weighing over 400 pounds.

• Tolerate standing and walkingfor up to 8 hours at a time on hard and soft surfaces.

• Frequently perform activitiesthat require stooping, bending and reaching, squatting, balancing, and trunktwisting throughout the day.

• Constantly able toeffectively communicate.

• Occasionally required to sitin a stationary position for up to 30 minutes at one time.

• Frequently able to detect anddiscern both loud and soft sounds such as resident calls, verbal orders,monitors, and phone calls.

• Frequently able to use handsand fingers to perform simple and fine motor handling tasks, such as takingvital signs, writing, and using a keyboard.

• Frequently able to usedifferent types of visual perceptions to perform activities such as movingmedical equipment, evaluating resident/patients, reading monitors and charts,and reading computer screens.

• Occasionally required todetect unusual odors that could indicate infection or other problems with aresident, or environmental issues such as smoke.
