Title:  Director of Health Services

This executive responsibility is for the leadership strategy, operation, and performance of a specialized case management/utilization management health care delivery program.  Professionally focus on preventive health initiatives for the older adult to age in place lowering utilization in our premier retirement community. The program significantly impacts the financial security of the organization through life care utilization reduction in higher care settings.

The health service director oversee the coordination and administration of all program aspects including planning, organizing, staffing, and outcome based results. Provide direct oversight of programs and people ensuring seamless team management, development, program delivery, and evaluation.


•         Oversees, coordinates and monitors all health service team members’ activities to facilitate an integrated proactive utilization case management program.

•         Create annual program performance plans, goals, budgets and standards of operations.

•         Demonstrated ability to communicate, problem solve, and work effectively with people.

•         Skilled at supervising high functioning teams.

•         Able to use effective management principles.

•         Excellent organizational skill with the ability to manage multiple priorities.

•         Work independently and handle multiple projects simultaneously.

•         Functions as hands-on supervisor for daily case management activities including medical necessity review, initial assessments, continued stay review, discharge planning, and other duties as required.

•         Achievement of department quality standards. 

•         Maintains effective team member relations.

•         Support and develop philosophy, policies and procedures.

•         Manages and completes assigned work plan objectives and projects in a timely manner.

•         Assists team members in improving skills, creativity and problem solving.

•         Acts as an information and problem solving resource for health service team members.

•         Achievement of department quality standards. 

•         Analyze hospitality, health, wellness and supportive services needs of residents at Alexian Village, and develop and implement services to meet those needs.

•         Plan and develop innovative programming and models of service that will meet the needs of older adults’ independent living settings.

•         Maintain effective intradepartmental and public relationships.

•         Accountability for the daily census and operations for the fitness center that specialized in personal training services to older adults and the spa program that specialized in services for older adults.

•         Triage after hours call service for urgent health care needs.

Job Requirements


Education:  Bachelor’s Degree in Social Worker, RN, LPN, Psychology or Related Field

Supervises:     Wellness care coordinators

The Fit for Life program including all personal training staff

Primary Responsibilities:

1.      Recommend and maintain the departmental budget.

2.      Develop and analysis systems that provide care coordination services for independent residents wishing to maintain and/or regain their independence.

3.      Facilitate transfers of residents to and from their apartment through all levels of care.

4.      Prepare monthly and other statistical reports.

5.      Maintain effective interdepartmental relations.

6.      Pursue research activities that support results of department activities.

7.      Support AVM philosophy, policies and procedures.

8.      Other duties as assigned by supervisor.
