Functions as a liaison between referring facilities and community admissions as requested, and is responsible for determining the appropriateness of skilled/rehab or long-term care patients/residents in terms of medical stability, diagnosis, impairments, functional ability, willingness to participate, and social and financial considerations. Also provides financial updates as needed throughout the admission process. Works in concert with other admissions and clinical team members to schedule candidates for admission to the appropriate level of care at the community. Provides guidance and direction for all individuals referred to community location.

 Evaluates prospective patients and residents for skilled care and long-term care. Obtains all necessary intake information. May be asked to provide admissions input for assisted living and memory care at Ascension Living (AL) community locations. Evaluation initiated within two hours of receipt, weekends excepted, with decision reached by 24 hours.
 Assessments for admission and all referral data must be completed promptly and decisions made within a time determined by ministry leadership. Must make critical clinical decisions and work with referring physicians and case managers to gain more information as needed including on-site evaluations as required.
 Collaborates with clinical staff, therapists, social/discharge staff, patient and family, and physicians as appropriate for accurate assessments and placement.
 Maintains ongoing communication regarding status/progress of candidates, between referring sources and community staff on a daily and ongoing basis.
 Provides alternative service recommendations for candidates not meeting admission criteria, to referring sources.
 Assists with annual Policy/Procedure review/update for admissions process.
 Participates with manager to improve services and provides on-going support to clinical staff for additional information or resident data.
 Represents department in daily huddles, staff/interdepartmental meetings as requested. Makes reports for the manager as requested. Documents involvement. Participates with manager in departmental planning sessions to improve services.
Represents Ascension Living within the community to provide information, education, and promote positive relationships at all times.
 Documents resident and family needs and communicates information to necessary parties.
 Provides after-hours admissions as needed and education to external sources about the services provided by the community.
 Other duties as assigned

Job Requirements

 Two years experience in Skilled Rehab and/or Long Term Care required. Previous Acute Care-Med/Surg experience preferred. 
 Bachelor’s degree in Nursing from four-year college or university preferred.

 Current Registered Nurse (RN) license  in the state where employed required.
